Network: Check you have the right type of equipment to fit your business requirements in the first place and whether your network is configured correctly to produce the best performance and has the resilience to cope with business demands. 

Security: Its essential that your company has security policies in place to meet governance and compliance requirements, as well as having a business continuity and disaster recovery plan.

Personnel: Are people doing what you actually pay them to do? In small IT departments people often have to multi-task. Someone may be hired as a developer, but end up doing a little bit of everything and becoming a generalist.

Purchasing: Make sure your IT purchasing follows purchasing rules for the rest of the company. So if its policy to get three quotes for work, then make sure IT does the same.
